Think of ferritin as your iron savings account and hemoglobin as your iron paycheck. You can drain the savings account substantially before the paycheck actually bounces. That gap between “savings are low” and “paycheck bounced” is exactly the stage doctors call iron deficiency without anemia, and it’s actually quite common, showing up in people who feel unwell despite a normal complete blood count.
Why Are There So Many Different Ferritin Numbers?
This is where most people get confused, so let’s break it down simply. There are three separate figures in circulation, and they answer three separate questions.
The World Health Organization sets a deficiency cutoff of below 15 µg/L in adults, mainly designed for population level use rather than individual diagnosis.
In everyday clinical practice, a threshold of below 30 µg/L is commonly used to diagnose iron deficiency, since it helps catch more cases, including both iron deficiency with and without anemia.
Your lab report will usually print a separate “normal reference range,” often roughly 20 to 250 ng/mL in men and 10 to 120 ng/mL in women. This is a population spread, not a deficiency line.
In practice, this means someone can have a ferritin of 25 µg/L, sit inside their lab’s “normal” range, fail the WHO’s strict population cutoff, and still meet the more commonly used clinical threshold for iron deficiency. All three statements can be true about the same blood test. That’s why the number alone doesn’t tell the full story, and why it needs to be read alongside your symptoms and, often, a hemoglobin result.
Can You Have Low Ferritin Without Anemia?
Yes, and this is worth understanding on its own. Iron deficiency without anemia happens when your ferritin has dropped but your body is still managing to produce enough healthy red blood cells to keep hemoglobin in the normal range. Some people feel completely fine at this stage. Others notice fatigue, hair thinning, or restless legs even though a standard blood count looks normal, which is exactly why ferritin is worth testing directly rather than relying on a complete blood count alone.
How Do Doctors Tell the Two Apart?
The distinction comes down to which tests are run and what they show.
A complete blood count checks hemoglobin and red blood cell size, helping identify anemia.
A ferritin test measures your iron stores directly, often dropping before anemia appears.
In people with inflammatory conditions, ferritin can run artificially high even when iron is genuinely low, since ferritin also behaves as an inflammatory marker. In these cases, doctors often add a transferrin saturation test to get a clearer picture.
If both ferritin and hemoglobin are low, that’s iron deficiency anemia. If ferritin is low but hemoglobin is still normal, that’s iron deficiency without anemia, an earlier and often more treatable stage.
Does the Treatment Differ?
Largely, no. Both are typically treated with dietary changes, oral iron supplements, and addressing whatever’s causing the iron loss in the first place, whether that’s heavy periods, gastrointestinal blood loss, or inadequate dietary intake. The main practical difference is urgency and monitoring. Anemia usually prompts closer follow-up, since a genuinely low hemoglobin has broader effects on oxygen delivery throughout the body.
